ASCDI Membership

APPLICATION FOR MEMBERSHIP - You can fill out the Application below or print this form to Print an application and FAX it to us in the US at 561-266-9017. Annual dues in the ASCDI are US$900. There is a one time Application Fee of $250US. After we recieve your application, we will invoice you for that fee, do a background check and submit your application to our Members for a vote. Once it is approved we will invoice you for the balance of the year.

By submitting this Application, you are agreeing to abide by the ASCDI Code of Ethics. Click here for more information on the Code of Ethics. Those found guilty of the Code of Ethics face disciplinary action, non-admission and/or expulsion from the Association. By submitting an application you are agreeing to waive all legal remedies you may have against the Association or its Members for such action.
